Immigration physical exam have quite few more requirement compared to regular physical exam, for instance in regular physical exam you do not need necessarily to be tested for Tuberculosis, unless you have medical conditions that makes your immune system weak, or some other requirements. But for Immigration it is required by all applicant to do it. another difference is that sometimes specific tests are required for screening of certain diseases, that normally will not be required in regular circumstances. We need documentations of any vaccinations done, if it is done in another country you need to bring a copy of that which is translated in English.
